If you had difficulties getting the SUBMIT function to respond, first, as there is no feedback nor acknowledgement to go by, you need to check your sent folder for a possible success. You may in fact have succeeded?
If that doesn’t work I can send you the two questionnaires as PDF files which you can complete and send as attachments to “geographicnames@ontario.ca”
